Over the last 5 years, Yorktown Family Services has operated a youth-specific hub that provides a wide range of services that includes Youth Outreach Services, Youth Mentorship, Youth Groups and Groups for young adult parents along with various social service programs. In partnership with Unison Community Health Centre, we are pleased to announce that starting Tuesday, September 18th, 2018 the West Toronto Youth Hub will be launching a series of new programs starting with a counselling walk-in clinic that will open two days per week. Please see the attached flyer for times and more details. Over the coming weeks, Reconnect Community Health Services, WestReach and partnerships with various healthcare providers will launch to deliver one-stop services for youth and on the spot seamless transitions to adult mental health services and primary health.Â
