Care Navigation

The West Toronto Youth Wellness Hub Care Navigator supports youth and their caregivers encountering complex and multiple mental health challenges in accessing services both internally within the hub and externally with partner organizations.

The Care Navigator supports service coordination and transitional needs from one service to another within the Hub and between primary healthcare environments both internal and external i.e. Hospitals, Community Health Centers, Family Health Care Teams.

This is an integrated care model that consists of a multi-disciplinary team that is mobilized by the Care Navigator to respond to the needs of youth and their caregivers in a comprehensive and efficient manner.
