RBC Foundation: Supporting youth mental health through Yorktown’s Care Navigation Program

 

RBC Recognizes that supporting the mental health of Canadian youth is fundamental to the success of our communities and our collective future. RBC has invested in Yorktown’s Care Navigation program, considering it a proven vital service in our community and a means to help youth access timely and quality mental well-being resources. Yorktown has honoured RBC Foundation through our Community Partnership Award for its support of the Care Navigation program during the 2020/21 fiscal year.

RBC Foundation’s funding of Care Navigation has enabled Yorktown to advance sector collaboration between primary health care services and community-based mental health care. An innovative integrated care approach, Care Navigation results in decreased hospital visits, sustained positive mental health outcomes, and seamless transition of youth and their families to ongoing care without disruptions. Youth that access Care Navigation experience minimal barriers as a result of collaborative processes with internal and external services.

We thank RBC for supporting our vision with the Care Navigation program and continuing to be invested in systems that respond to the growing and changing needs of our youth communities.
